Job summary

The Intermediate Care at Home Team (IC@home) and Community Inpatient team (CIU) provide short term reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. We encourage and support our Service Users to live as independent lives as possible and we always act in a manner that supports the care, protection, health and well-being of the Service User. This post will be split between the two services to help improve integration and joint working supporting patients in their reablement journeys.

Hours: 37.5 hours per week Monday to Friday

Salary Range 6 £33,675 - £40,439 per annum

We are seeking a highly motivated and passionate physiotherapist to join the therapy team at Care Plus Group. The post will be split between two services, Intermediate Care at Home Team (IC@home) and Community Inpatient team (CIU). Both services provide short term reablement for Services Users who may need to regain their confidence and skills on discharge from hospital or to get over a short-term health exacerbation. We encourage and support our Service Users to live as independent lives as possible and we always act in a manner that supports the care, protection, health and well-being of the Service User. This post will help improve integration and joint working between the two services supporting patients in their reablement journeys.
