Job summary
Palliative and End of Life Care Services provide various of levels of support to people with limited conditions and those who are in the final phase of life. The service provides, one to one support as well as advisor capacity to other professionals.
The service will work through a central point of referral, working in conjunction with the Community Nursing Teams.

Hours: 30 hours per week

Salary Range 3 point 10 £21,414 pro rata per annum fixed salary point
You will provide a comprehensive administrative support service at any location across Care Plus Group under supervision. You will exercise initiative and independent judgment with minimum supervision, ensuring that you exercise a high degree of tact, diplomacy and confidentiality at all times. You will also take referral information from professionals.
