We are looking for a new member of the dementia crisis team, providing care and therapeutic intervention to patients in a range of community settings under the guidance of professionally qualified members of the team.

Main duties of the job

The post holder should have a passion for assisting disabled and/or vulnerable adults to live at home with dignity and independence while supporting them by providing rehabilitative care.

The post holder would need to undertake a range of delegated clinical patient care duties in a range of settings, to maintain patient records.to assist in the ongoing development and evaluation of the service
