Job summary

We have a rare opportunity for a part time palliative care advanced clinical practitioner 8a (21hrs per week) based at Wisdom Hospice, Rochester.

We are looking for a clinical leader who is passionate about supporting patients and their loved ones through their end of life journey, enabling the best experience. You will be an autonomous practitioner who thrives on expert clinical decision making and works within the 4 pillars of advanced practice'

Main duties of the job

The post holder will work as an autonomous practitioner within the specialist palliative care service. The role involves a high degree of autonomy and complex decision making. This is underpinned by a master's level award or equivalent that encompasses the four pillars of advance practice:

clinical practice

leadership and management

education

research and audit

The post holder will have the ability to manage the patient's complete clinical care in partnership with individuals, families and carers. The post holder is expected to be able to complete a complete medical and holistic assessment of the patient. Subsequently they will be expected to make decisions regarding diagnosis, interventions and prognosis in a context of complexity, uncertainty and varying levels of risk, being accountable for their decision making thus managing the complete care of the patient.

The ability to manage the patient's complete clinical care includes assessment, diagnosis and management of reversible conditions, management of pre-existing long-term conditions as well as the assessment, diagnosis and management of the presentation directly linked to the end of life. This is supported by a wide scope of practice with regard to prescribing.

The post holder will be expected to participate in the medical weekend/ bank holiday on call rota.

For RCN registrants this is achieving credentialing for RCN Advance level Nursing practice and for non-RCN registrants evidence they meet the credentialing criteria.

Once a nationally agreed multi professional register is agreed this will become the standard requirement.
