We are looking for someone with:

  • A passion for caring for patients with tissue viability challenges.
  • Experience of caring for patients with wounds of all types, including pressure and leg ulcers, acute and chronic wounds, diabetic ulcers, burns etc.
  • Able to demonstrate sustained professional development and a willingness to continue this with a focus on tissue viability.

The successful applicant will be working in close collaboration with all members of the tissue viability team to provide comprehensive, evidence-based tissue viability/wound care advice and support to nursing staff and other healthcare professionals within Medway Community Healthcare and to external organisations and agencies.

The tissue viability team also provides extensive training and resource on all aspects of tissue viability and the successful applicant will be expected to perform this training both 1-2-1 and, in a group, setting. The applicant will be expected to triage clinical referrals to the service for advice and prescriptions for specialist equipment.

Main duties of the job

Job purpose

  • To work in close collaboration with all members of the tissue viability team, to provide comprehensive, evidence based tissue viability care for Medway Community Healthcare
  • To assist the tissue viability team in working across all community settings including wound clinic, nursing homes, patients own homes, residential homes, local prisons as required
  • To carry out pressure ulcer risk assessments, wound assessments and lower limb assessment inclusive of Doppler's to create appropriate pathways of care for patients referred to the tissue viability team.
  • To provide advice on pressure relieving equipment prescriptions to ensure patient safety.
  • To lead on caseload management of the tissue viability caseload.
  • To undertake duties delegated by the tissue viability and wound therapy specialist nurse.
  • To assist the tissue viability and wound therapy specialist nurse to lead, develop and monitor clinical standards of care and policies relating to the management of pressure ulcers, leg ulcers and other wounds.
  • To support the implementation of policies and guidance related to tissue viability.
