The Medway Peninsula PCN are looking to appoint a highly specialist physiotherapist who can work at an advanced level of practice within a PCN, utilising their in-depth knowledge, clinical skills and experience at a senior level, in the management of patient's musculoskeletal conditions.

Main duties of the job

  • Under the direction and supervision of the Head of Service for Adult Therapy to undertake a an FCP role in a local PCN alongside a small team of FCP clinicians.
  • To act as a source of expertise on the management of musculoskeletal conditions providing an advisory service to primary care staff including physiotherapists, podiatrists, practice nurses and the GPs. To act as a link with community services and secondary care professionals including Consultants, MSK triage/Clinical assessment service clinicians & Physiotherapists.
  • Is an autonomous practitioner to undertake highly specialist assessment and treatment of patients within the musculoskeletal speciality who may have highly complex acute and / or chronic presentations.
  • Through the application of advanced clinical reasoning and differential diagnosis skills responsible for the formulation of accurate clinical diagnoses and physiotherapy treatment plans advising the patient, carers/ relatives and other AHPs appropriately.
  • To work with other senior staff within the PCN to ensure that Clinical Governance targets are met including the development of clinical standards and protocols, audit, research, clinical and non-clinical risk management using NSFs, NICE guidelines and evidenced based practice.
  • Responsible for prioritisation/ triage of patient referrals, identifying any serious pathology and managing appropriately.
