Job summary

- Do you have an NVQ level 3 in Health and Social Care or equivalent?

We are looking for flexible band 3 assistant practitioners with excellent people and administrative skills to work within our team. If you have the skills to care for our patients with respect, dignity, empathy and warmth we would like you to be part of our caring practice nurse team and make help make a different to people living with serious mental illness and dementia.
Main duties of the job
- To work as a member of the GP Mental Health Practice Nurse team, undertaking and completing physical and psychological health checks for patients on the GP Serious Mental Illness Register in a range of community settings under the guidance of professionally qualified members of the team.
- Support the team with a range of administration tasks including contacting patients by telephone, completing documentation and making appointments
- Act as a liaison/coordinator on behalf of the team
- To maintain patient records
- Assist in the ongoing development and evaluation of the service
- This role includes driving and use of VDU
