Your Healthcare CIC

Clinical Governance, Quality Improvement & Patient Safety Facilitator

The closing date is 12 April 2026

Job summary

Are you a senior nurse with a strong understanding of, interest in and experience of governance, who is passionate about improving patient outcomes and experiences in an inclusive way? Are you highly organised, proactive, flexible, focused and approachable, with very high quality verbal, written and presentation skills? Are you able to balance a range of different workstreams whilst maintaining exceptional attention to detail? When things go wrong, do you focus on how systems can be improved rather than individuals, and do you have a proven track record of successfully applying this systems based approach?

You will work within the Service Effectiveness Team, collaborating directly with the Lead to promote continuous quality improvement. This will support the organisations compliance with the requirements of the NHS Patient Safety Framework and the regulatory requirements set out by the Care Quality Commission. You will be responsible for supporting the ongoing delivery and strengthening of the organisations Patient Safety Plan and will provide clinical scrutiny, effective support and a skilled and experienced perspective to the already established YH incident reporting and management framework. You will also ensure robust service user engagement is embedded into improvement activity, supporting services to implement meaningful and workable patient and service user centred changes developed collaboratively with YH Members, our Patient Safety Partners and the community.

Main duties of the job

Support organisational CQC readiness and help ensure compliance with all relevant governance, assurance and regulatory requirements.

Provide clinical oversight to patient safety incident reporting, review, learning responses and safety actions, promoting a non blame, learning focused culture.

Contribute to the development, implementation and monitoring of policies, ensuring they are meaningful, accessible and aligned with national standards.

Support the delivery of quality improvement activity, helping services identify issues, measure impact and implement sustainable, system focused improvements.

Ensure inclusive and robust service user engagement is embedded across governance and improvement work, informing service development and decision making.

Work collaboratively with teams across the organisation to provide a skilled and experienced clinical perspective, supporting safe, effective and patient centred care.

Support the generation of clear, accurate information for internal and external governance reporting, including progress updates, themes and learning.

You will be central to strengthening a culture that maximises what goes right, rigorously identifies and addresses system vulnerabilities, and continuously reduces the risk of things going wrong across the organisation.
