Job summary
An
exciting opportunity has arisen for a highly motivated and enthusiastic HCPC
registered occupational therapist to join our dynamic and supportive team at
Your Healthcare CIC.
This
brand new post is joint funded between Your Healthcare and Kingston and
Richmond Foundation Trust and is ideal for someone with both hospital and
community experience, but with a desire to support us in developing this new
service moving care from the hospital to the community. Team support will be available, and Your
Healthcare has a protected CPD budget which all employees can apply for.
If
successful, you will in-reach into Kingston Hospital working alongside the
Frailty team to identify patients who can leave hospital with a home first
approach and take them home to assess in their own home. You will work closely with community health
and social care teams to prevent hospital admission, expedite discharge and
provide care at home.
You will be mostly based at Hollyfield
House/Kingston Hospital although you will also need to be able to travel around
the borough of Kingston and to our other sites as needed.

Main duties of the job
This is a fixed term post and is for 37.5 hours per week. The usual working hours are 9-5 and will be Monday Friday. This role may be required to work weekends/unsocial hours.
We are looking for someone with:
Diploma/degree
in relevant Occupational Therapy.
HCPC
registration
Evidence
of relevant Continuing Professional Development
2-3
years post graduate experience in field of rehabilitation (Hospital and/or
community settings)
Competencies
of Band 7 gateway.
Experience
in Assessment and Rehabilitation in a wide range of conditions found in the
adult population.
Experience
of clinical supervision of junior staff and students.
Excellent
oral and written communication skills, report writing and Computer literacy,
Information Technology Skills.
Ability
to plan and prioritise own caseload, organise plan and execute complex
discharges.
Working
knowledge and understanding of Integrated Governance and the implications for
services, including audit, risk management, health and safety etc.
Current
driving licence and access to vehicle for work purposes
