Job summary

We require nurses with community experience to work in the Twilight community nursing team. This is a bank position so shifts would be on an ad hoc basis as the service requires. Hours are 7pm till Midnight. You will work as part of a team of 4 staff made up of nurses and support workers lead by an experienced Community Nurse.

This is a bank post. The usual working hours are 19:00 til Midnight (5 hrs shift)

You will be based at Hollyfield House and then travel to SU homes to deliver care, so access to own car is required. Milage is paid as per AfC T&C. As this is evening work, you will work in a pair for safety.

Main duties of the job

To provide and maintain a high standard of skilled nursing care for patients in their own home, in compliance with protocols and guidelines.

To assess and re-assess patients with non-complex conditions, reporting back to the Twilight Senior Nurse and /or District Nurse on any interventions undertaken.

To respond effectively to the local practice population health needs through working within the primary health care and social care team.

Promotes health in all age groups by advising and supporting clients and families.

Communicates observations of physical, mental and social conditions to the Integrated Team.

Participates in health promotion activities in line with Your Healthcare and local priorities.

Participates in Clinical Governance activities such as clinical audit and research.

Complies with current Evidence Based Guidelines, Your Healthcare and relevant RBK policies and procedures.

Demonstrates evidence based clinical interventions to pre-registration students while on placement with the nursing team.

Reports to the Senior Nurse any accidents, incidents or near misses to own person, the patient, patients property and equipment, in line with current policy
