An exciting new opportunity has arisen for you to join the Urgent Care & Support

Service at Your Healthcare as a Clinical Digital & Practice Educator. Working in the community supporting current digital integration projects implemented in care homes, designed in line with the NHS strategic goal of implementing the Enhanced Health in Care Homes Framework.

The post holder will work closely with the care homes in the London Borough of Kingston, to provide clinical education, support, and guidance to care home staff and support and train care home staff on the use of digital equipment and to champion best practice in the use of digital care technology; promoting digital integration and digital inclusion, ensuring care staff are making the best use of digital care technology within CQC registered care homes in Kingston.

The post holder will support care home staff to achieve the appropriate clinical competencies, knowledge and skills and embed into practice these assessment, escalation and communication approaches including those that the digital care technology enables.

The post holder will be required to develop ways to present information that overcomes barriers to understanding, such as developing motivational, persuasive and negotiating skills to deliver training, maintain relationships and communicate and gain the cooperation of others. Much of the information will be extremely complicated such as communicating digital language that may be new to the teams.

Overview of The Urgent Care and Support Service

The Urgent Care & Support Service (for care homes) in the borough of Kingston is part of the Urgent Community Response service, working to reduce hospital admissions from Nursing, Residential and Learning Disability care homes. The team responds to urgent care visits providing advanced clinical assessment, diagnosis and management including clinical support and education in combination with a dedication to building a network of health and social care professionals working together in improving proactive care for residents.
