Job summary
We are looking for Health Care
Assistants to join our highly skilled and caring Twilight team.
You would be working with a qualified
nurse to help deliver care to clients in their homes in the evenings.
Shifts are 5hrs (19.00-00.00 in the
week and 18.00-23.00 at weekends).
Experience of working as a carer is
essential and community experience is desirable.
The service works from 7pm til midnight
7 days per week.
Access to a car and ability to drive
are required for this role.

Main duties of the job

Job responsibilities
- As a continuation to the district nursing services to work in support of the senior community nurse to provide high quality skilled nursing care in the evening hours.
- Working from 7pm til midnight under direction of senior community nurse.
- Contributing to the ongoing assessment, planning, implementation and evaluation of the holistic care needs of the patient, family and/or carer.
- This role supports the District Nursing Team in two localities across the Kingston borough in the delivery of integrated, person centred, responsive, co-ordinated and high quality care.
Key Result Areas
Carry out a range of duties under the direction of a
registered community nurse in accordance with policies, procedures and
guidelines including:
Urinalysis
Simple wound care
Blood glucose monitoring
Temperature and pulse recording
Blood pressure monitoring
Setting up of PEG/RIG feed
Health promotion
End of life communication
Health education
Communication and working relationships
To build a professional rapport with patients and their
families, communicate with them in a way that respects their views, autonomy
and culture.
To communicate effectively with all professionals concerned
with the patients care.
Ensure patient confidentiality is maintained at all times.

maintained and necessary equipment is prepared at the beginning of the shift
Undertake clerical duties as required Using Carenotes
patient record system to admit and discharge patients and to maintain diary
sheets for each shift
Professional development - To undertake relevant activities
to meet the training objectives identified with your supervisor.
Undertake mandatory training.
To keep a record or your training and clinical development
activities.
Clinical governance - Participate in clinical governance and
quality improvement projects as allocated by your manager.
Personal
management - To exercise good personal time management, punctuality and
consistent reliable attendance.
