We are a dynamic, friendly, and supportive team where all members are encouraged to contribute to the operation and development of multidisciplinary team (MDT) working in the Kingston and Richmond Boroughs

The Locality Coordinator Team coordinates and administrates the monthly and fortnightly GP practice-based MDT meetings at each of the practices across both Kingston and Richmond. This Care Home MDT Coordinator position is vital to the roll out of a proof of concept, delivering MDTs in select practices in Kingston and Richmond.

We are looking for an enthusiastic individual who can demonstrate initiative, professionalism, and a strong team-working ethic. The successful candidate will be able to work independently when required, work effectively with a wide range of health and social care professionals, and contribute to improving MDT working as part of the team.

Main duties of the job

The post holder will also maintain a wide range of administrative tasks to a high level and keep up with the changing demands of the service.

You will be competent and confident in the use of IT systems/platforms used within Your Healthcare. You will welcome the opportunity to take on new challenges and thrive in a busy and demanding environment. You will have the ability and confidence to problem solve and self-learn and enjoy working independently as well as part of a team.
