Job summary
We are looking for a highly specialist
dietitian to continue to develop and provide evidenced based, collaborative and
effective clinical pathways, interventions and processes for a number of key
health conditions and presentations prevalent within the current dietetic
learning disability caseload (Diabetes prevention / weight management). The
postholder will work alongside our band 6 dietician and will focus on delivering
outcomes for specifically agreed health conditions as well as offering more
specialist support for those on the dietetic caseload presenting with highly
complex nutritional needs. The successful candidate will also be responsible
for setting up and running an exciting new health and well-being intervention
that has been designed to meet the specific needs of people with learning
disabilities (Based on the Pentathlon).
Main duties of the job
The Neurodevelopment (NDS) service provides
a service for adults with learning disabilities living in Kingston and Richmond
boroughs. You would work independently in the community but also be an integral
part of a well-established and dynamic multi-disciplinary team. We can offer
flexibility in working schedule (15 hours a week).
As a highly specialist clinician within
the Neurodevelopment al service you may also be asked to contribute more
generally to multidisciplinary interventions and developments provided by our
service and so excellent communication and clinical skills are required as well
as emerging leadership skills.

3. Dimensions
This post will develop and offer highly specialist interventions and some supervision or support for an established dietetic post within the Neurodevelopmental service for Your Healthcare.
The post will contribute on the whole to the activity of the Neurodevelopmental specialist healthcare service for people with learning disabilities working across Richmond and Kingston boroughs.
4. Key Result Areas
4.1. Clinical
- To provide a highly specialist dietetic service to the Neurodevelopmental service for adults with learning disabilities
- To provide a community service in liaison with other related dietetic posts in partner organisations (e.g. Kingston Hospital, Home enteral feeding services, NUTRICIA, GPs)
- To communicate effectively with patients and carers to maximise advice taken by the patient. This includes, where necessary, using interpreters for patients where English is not a first language and methods of communication may need to be changed where patients have limited ability to understand
- To communicate with referring agents and other agents as necessary such as nursing staff, therapists and social workers through personal, written or telephone contact
- To undertake comprehensive assessments of all patients diets and lifestyles to formulate individualised management and treatment plans. This will include anthropometric measurements, interpreting biochemical data and evaluating nutritional information using quantitative and qualitative methods including in the field of enteral feeding
- To attend and contribute to team meetings and case conferences as necessary and to take an active role in the Neurodevelopmental Service complex care pathway.
- To work autonomously with day/day support and line management from the Principal speech and language therapist.
- To provide domiciliary visits to patients as required
- To accept clinical responsibility for the dietetics patient caseload and to organise this effectively with regard to clinical priorities and use of time.
To be responsible for the updating of diet sheets, information and other literature for the care of patients within the Neurodevelopmental service.
- To maintain accurate and up to date patient records which meet the department standards.
- To enter information onto data systems (e.g. Advance care Notes) as detailed by the immediate line manager and to maintain an effective data collection system.
4.2. Service Delivery
- To initiate and contribute to the development of policies and protocols for the Neurodevelopmental service in the community that will impact on all service users.
- Ensure that any future designated Dietetics staff implement policy and service development changes.
- Deliver services in line with Better Standards for Health Care and relevant legislation.
- To provide services in a variety of community sites as required by service models used and to be able to travel between sites using suitable transport.
- Along with other clinicians in the Neurodevelopmental service, draw upon specific professional knowledge and skills to establish effective matrix working
- Support multidisciplinary team working, by facilitating colleagues to effectively prioritise, plan and manage their caseloads
- Contribute to the development and implementation of guidelines, protocols and patient care pathways for use across the Neurodevelopmental service
- Be responsible for ensuring the effective selection and use of all treatment resources available.
- Be actively involved in the collection of appropriate data and statistics for the use of the service.

- Work under direction of the principal speech and language therapist to train and supervise more dietetic junior staff, technical instructors, assistants and students as required. This may include contributions to formal appraisal documents.
- To cascade information to staff within area of responsibility.
- To participate in clinical supervision and through the supervisory framework, support other junior staff in effective decision making with regard to caseload management
- To attend any team briefing meetings and other meetings as requested by the Principal Therapist and provide input in relation to dietetics on day to day issues and clinical matters as indicated.
- With other members of the Multidisciplinary Team contribute to the prioritisation of referrals to the Neurodevelopmental service
- To promote and develop quality improvements through supervision, audit and other CPD activities as appropriate to the needs of Dietetics as part of the Neurodevelopmental service
