Job summary

The community Rapid Response Team is a well-established admission avoidance service which is part of the 2-hour urgent community response (UCR) service.

We are looking to recruit an enthusiastic and motivated nurse, paramedic or physiotherapist to work in the Rapid Response team based at Hollyfield Road, Surbiton

The service provides a 2-hour response to adults who are experiencing a health crisis and require assessment and intervention to support them in their own home, rather than being admitted to hospital. The role also involves working collaboratively with multidisciplinary professionals to manage complex health and social care needs.

Main duties of the job

This is an exciting opportunity to enhance and develop a wide range of skills including physical assessment and clinical decision-making in a community setting.

The ideal candidate should be able to plan and prioritise their own workload, be confident in independent decision making, with an awareness or experience of physical assessment.

The position would suit an experienced clinician who is looking to develop skills in advanced clinical and autonomous practice. Appropriate training, development and support will be provided.
