Main duties of the job
The community rapid response team is part of the 2-hour Urgent Community Response service. This is an admission avoidance service for people in the community experiencing a sudden deterioration in health and wellbeing. We are seeking to recruit an enthusiastic experienced nurse or allied-health professional, who is able to work autonomously in the community. They must be able to demonstrate advanced clinical skills, including physical assessment, clinical decision making and differential diagnosis for a range of common acute and chronic conditions.
Candidates must also have an interest and experience in service development and leadership.
The role involves visiting people in their own home, who require urgent assessment and intervention with a view to preventing unnecessary hospital admission. The role also involves working collaboratively with multidisciplinary teams to manage complex health and social care needs.
