Are you looking for a new and exciting opportunity as part of an Integrated Network Team supporting patients in their own homes?

Sirona are looking for dynamic Assistant Practitioners to support delivering clinical and therapeutic care, within a comprehensive competency framework.

We have vacancies across numerous locations:

North & West Bristol

Inner City and East Bristol

South Bristol

Woodspring

Weston, Worle & Villages.

You will work in a team with other multi-disciplinary professionals including Nurses, Physiotherapists, Occupational Therapists, Administrators and other support staff, on a rota system between the hours of 8am-8pm over a 7 day working period.

A full driving licence and access to a vehicle is required for this role and whilst we are looking for full time hours, we can consider part time hours.

Main duties of the job

Clinical monitoring of acutely unwell patients to support prevention of admission to hospital when clinically safe to remain at home.

Support the early discharge of patients from hospital: including providing holistic initial assessments of functional, mobility and medication needs.

Carry out the relevant clinical competencies required of the post e.g., wound management, administration of medications including insulin to non-complex patients (under direction of registered nurse), catheterisation (supra-pubic/male/female), equipment and mobility assessment, and functional assessments as outlined in Sirona Assistant Practitioner competency framework

To undertake and complete appropriate training and competencies in order to carry out the role of Assistant Practitioner.

Essential

The ability to carry out the relevant clinical competencies required of the post: clinical, nursing and therapy based.
Able to assess patients needs using EMIS templates, assessment tools and produce treatment plans, communicate and record clearly and succinctly, verbally, in writing and electronically using varying IT systems following appropriate training e.g. E-mail, EMIS.
To demonstrate a sound understanding of Clinical Governance, Clinical Risk and to use Evidence Based Practice in order to provide optimum care to patients.
Experience of direct client care in a health or social care setting. Experience of team working.
Experience of working without direct supervision.

Desirable

Compression Bandaging, Phlebotomy, Wound Management, Urethral and Supra Pubic Catheterisation.
Experience of completing Therapy Assessments in the acute or community settings.

Qualifications

Essential

Successful completion of relevant Foundation Degree modules / Diploma in Clinical Health Care / diploma level 3 or equivalent.

Full driving licence and access to a car in work time (with business use car insurance).
