An exciting opportunity has arisen at Sirona care & health for a passionate and experienced clinician to step into a Band 7 Clinical Lead role within our Community Transfer of Care Hubs (CToCH).
This is a pivotal leadership position at the heart of discharge planning across Bristol, North Somerset and South Gloucestershire. We work within a fast-paced, multidisciplinary and multi-agency environment, where you will champion the 'Home First' ethos -- ensuring people are supported to leave hospital safely, promptly and with the right care in place.
Our Transfer of Care Hubs unite acute trust teams, community health professionals, Local Authorities and voluntary sector partners into one collaborative team. Together we focus on streamlining complex discharges and improving patient flow.

Main duties of the job
Clinical Practice
- Provide specialist clinical leadership in the assessment and case management of individuals with complex needs.
- Oversee clinical triage, ensuring referrals are directed to the most appropriate Discharge to Assess pathway.
- Use advanced clinical reasoning to guide safe and timely discharges, including for patients with complex comorbidities, mental health needs, long-term conditions or social challenges.
- Apply expert knowledge of the Mental Capacity Act, best-interest decisions and risk-balanced discharge planning.
- Work in partnership with acute, community and social care colleagues to deliver high-quality coordinated care
Leadership & Management
- Support and develop CToCH staff through clinical supervision, training and mentorship.
- Play an active role in Transfer of Care Hub leadership meetings and daily operational processes.
- Use data, KPIs and audit outcomes to identify areas for improvement, challenge long-term pathway decisions and enhance service delivery.
- Ensure effective caseload management and appropriate use of BNSSG rehabilitation capacity
Education & Quality Improvement
- Deliver training on discharge pathways to a range of multidisciplinary partners.
- Maintain up-to-date clinical expertise across acute and community discharge processes.
- Lead audit and research activities relating to discharge pathways and implement findings into practice.
- Drive service development and innovation to improve patient outcomes and efficiency.
