Associate Practitioner - Childrens continuing care team

The closing date is 05 February 2026

Job summary

Do you want to make a real difference to the lives of children, young people and their families every day? Come and join the Lifetime Service, where compassion, inclusion and teamwork guide everything we do.

We are a joint Community Childrens Nursing and Psychology Service supporting children and young people with life limiting and life threatening conditions, as well as those who meet the Childrens Continuing Care criteria. We help families through every stage of their journey, enhancing their quality of life and providing outstanding care at home and in the community.

Our team is based at Kingswood Hub in South Gloucestershire and we provide services across BNSSG, with additional support offered in B and NES. We care for children and young people aged 0 to 19, ensuring they and their families feel supported, valued and understood.

We are looking for caring and motivated individuals who want to use their skills to make a genuine impact. You will work closely with families and colleagues, bringing ideas, learning new skills and contributing to a supportive and empowering team culture.

The role includes a mix of nights, weekends and office based working, giving you variety and opportunities to build strong relationships with the people you care for. We have both 35 hour and 27 hour per week posts available.

If you ready to be part of a service that truly changes lives, we would love to hear from you. Join us and help children and families live richer and more fulfilling lives.

Main duties of the job

You will work in the Continuing Care team to undertake clinical shifts usually working alongside the Lifetime support workers (LSW) in the child's home or school setting. Observe the LSW clinical competencies and undertake annual clinical assessments of their skills to enable them to undertake the clinical interventions to the children and their families who meet Continuing Care Criteria (Bristol, South Gloucestershire, North Somerset and B&NES).

You will undertake the line management of a small team of LSW's which includes having 6 weekly one to one meetings, performance development reviews and supporting attendance reviews in accordance with Sirona's policies and procedures.

You will also work alongside the nursing team to develop and provide training and assessments to groups of LSWs.
