Community Homeless Health Support worker

Sirona's Community Homeless Health Team supports individuals who are homeless, sofa surfing and in temporary and emergency accommodation. We help 'bridge the gap' and promote re-engagement with health care.

As a Community Homeless Health Support Worker you will be supporting registered health care professionals to deliver health care and support.

You will proactively engage with individuals who have identified health inequalities at a range of venues in local communities, helping them to improve their lifestyle and overall health.

Day to day, you will contribute to tackling health inequalities by working with the most vulnerable groups. Providing care to service users in Bristol, North Somerset and South Gloucestershire (BNSSG) to individuals who are homeless, vulnerably housed and those that have been discharged from hospital. The post holder will need to be flexible and cover all areas of BNSSG as needed

Main duties of the job

o To carry outreach plans as prescribed by registered therapists, in accordance with instructions and recognised training received. This will involve a level of autonomy and decision making using protocols as drawn up by the service.o Provide assistance to clinical staff in the assessment and treatment of patients on discharge from hospitalo Follow a prescribed care plan to support service users at home including support with access to GP surgeries, outpatient appointments, general health careo To participate and support in promoting healthy and independent living practices to patientso To work collaboratively with other organisations across different sectors of health and social care - for example St Mungo'so To carry out general administrative and non-clinical work to contribute to the functioning of the team such as photocopying, filing, liaison and telephone calls.
