Job summary

We have a Secondment/Fixed term opportunity within the Public Health Nurse response team. The PHN response team is a workforce of specialist community public health nurses within the BNSSG public health nursing service.

The PHN Response team provides information, support and access to interventions for families with children and young people, following an attendance to Bristol Children's Emergency Department /Minor Injuries Unit/Urgent Care Centres and other emergency departments across the BNSSG area. This unique service is designed to work together with system partners to deliver an integrated, evidence-informed service for children and their families as part of the Sirona public health nursing service. The service has a focus on primary prevention, partnership working and early interventions.

Within this role you will help empower parents to make decisions that affect their family's future health and wellbeing. The team also work in partnership with system partners in Urgent care settings to enhance communications between professionals and to provide clarity of the public health nursing role.

Main duties of the job

The PHN response team will be using a report generated through connecting care to inform them of children 0-19 within the BNSSG area that have attended an urgent care setting. They will then apply Filters to the report to identify the children in line with a triage tool who require further assessment and follow up.

The team will contact parents by phone when assessment/follow-up is required to offer information, support, signposting, or referral.

The PHN response team will also be responsible for building partnership relationships with all urgent care settings (Yate Minor Injuries unit, Bristol Urgent Treatment Centre, Clevedon Minor Injuries unit, Southmead Hospital, Bristol Children's Hospital and Weston General hospital) by offering fortnightly PHN liaison meetings.

The Team will be attending all the urgent care settings across BNSSG to maintain links and support the staff within the settings awareness around public health.

The team will be available on a rota, to see children within the emergency department at BCH when requested by ED staff and it is agreed that a public health nurse contact is required, to provide information and support within the scope of the PHN role.

Qualifications

Essential

  • Degree or Post Graduate Diploma in Specialist Community Public Health Nursing (SCPHN).
  • Current NMC Registration or be awaiting registration confirmation following successful completion of the SCPHN course.
  • Registered Health Visitor or School Nurse
  • Registered Nurse (Adult, Children's, Learning Disabilities, Mental Health) and or Registered Midwife

Desirable

  • Evidence of successful study at post graduate level
  • Teaching Qualification such as FLAP, Supporting Students in Practice

Experience

Essential

  • Understanding of National and Local policy relating to 0-19 year olds
  • Experience of line management/leadership.
  • An understanding of the roles and contributions of members of the health visiting and school nurse team at primary health care team and community level.
  • An understanding of the current and changing role of Public Health Nursing
  • Ability to communicate and relate effectively with children, young people and their families, adults and colleagues, using a variety of communication systems.
  • Ability to deliver Health Promotion/Education to children /young people and adults
  • Demonstrate proactive search and distribution of new evidence and research which informs public health practice
  • Experience of working in collaboration with other health care and service providers to ensure seamless delivery of services for local families and communities

  • Familiar with MECSH, ASQ, ELIM, C Card
  • Understanding of Microsoft packages such as Teams, word and excel.
  • Group facilitation skills - either with parents, students or colleagues
