Job summary

Are you looking for a new and exciting challenge? We are seeking an Associate Advanced Clinical Practitioner to join our supportive, friendly and forward-thinking team within the South Gloucestershire Locality.

You will work under the guidance of an Advanced Clinical Practitioner (ACP) to identify and case manage individuals with highly complex needs, who require intensive community support. Your focus will be on achieving and demonstrating overall improvements in health care for service users who are frail and have several long term conditions.

You will also work in partnership with the wider team, including ACPs within both Long Term Conditions and Urgent Care, and other colleagues throughout the locality.

The Locality consists of:

  • Advanced Clinical Practitioners
  • Associate Advanced Clinical Practitioners
  • Community Nurses
  • Community Therapists
  • Paramedics
  • Community Support Workers
  • Administrators

Main duties of the job

  • Provide a service that focuses on achieving and demonstrating overall improvements in healthcare for complex/frail service users.
  • Contribute to the clinical leadership to ensure high quality and harm free care.
  • Work collaboratively with the ACPs and INT Managers to ensure all staff competencies are up to date.
  • Undertake relevant external university based masters level courses to support development of the ACP role.
  • Support the ACPs with facilitating and delivering clinical training and education within the INT in managing those individuals with long term conditions.
  • Develop and maintain strong, communicative working relationships with hospital staff, primary care service providers and other stakeholders.
  • Work as an autonomous practitioner, actively use and promote shared decision making and empowerment of service users to improve self-management of their long term conditions.
  • Work in partnership with the wider multi-disciplinary team.
  • Lead by example and be a role model for the team.

  • Current registration with NMC/HCPC or other relevant professional body.
  • Degree level qualification or equivalent.
  • Evidence of ability to study at Masters level clinical qualification.
  • Evidence of continuous personal and professional development.

Desirable

  • Masters level clinical qualification.
  • Coaching / Teaching and Assessing qualification / experience.
  • Physical Examination and Diagnostic Reasoning Module (or equivalent) at Masters Level or willingness to undertake.
  • Non-Medical Independent & Supplementary Prescriber or willingness to undertake.
