Working as part of NHS@Home, UHBW NHS Trust, NBT NHS Trust and Sirona care
& health are collaborating to bring acute care to people in the comfort of
their own homes.
NHS@Home is a new model of care, the roles
within our service combine home visits and virtual monitoring, in keeping with
the new emerging transformation change to use a virtual ward model. Remote
technology and remote health monitoring is used where appropriate. A
virtual ward is a safe and efficient alternative to NHS bedded care that is
enabled by technology and home visits. This innovative approach is delivering
high quality care, safely and conveniently for people at home where they
would rather be.
Our service aspires to help people to stay
well and at home by facilitating self and support using apps, technology
platforms and medical devices. Our ultimate goal is to ensure as little time as
possible is spent in an acute hospital bed and reducing the need for admission
to hospital.
Main duties of the job
We have an exciting opportunity to grow our
team. We are looking for a Trainee Advanced Clinical Practitioner to join our NHS@Home
Service.
This is an exciting post to join our new,
emerging service as we look to expand our skill mix. This role will develop you
into a qualified Advanced Clinical Practitioner with the skills to see and
assess patients within the Frailty pathway of our service
Full mentorship and managerial support will
be provided by the service as we aim to deliver strong leadership, enthusiasm,
and a positive attitude towards change throughout the development of our new
venture. This role involved both work based and academic learning, contributing
towards gaining a masters degree in Advanced Clinical Practice from a Higher
Education Institute. This trainee Advanced Clinical Practitioner role will
develop knowledge and understanding of the Health Education England for pillars
of advanced practice.
You can either be placed on an Advanced
Practice Apprenticeship (commencing date TBC) or can build on existing modules.
After 18 months-2 years, you would complete the Advance Practice credentialling
process (aligned with HEE) to move to an 8a Advanced Clinical Lead role.

- Experience of emergency / urgent care service user assessment and planning / designing emergency care plans.
- Experience of using clinical examination and diagnostic reasoning skills in emergency care.
- Experience in a relevant Primary Care and or Pre-Hospital and or Acute area of practice.
- Ability to work with a high level of autonomy and shared decision making.
- Ability to effectively communicate complex clinical information.
- Ability to prioritise against tight deadlines and work under pressure.
- Ability to effectively motivate others.
- Evidence of up-to-date clinical skills relevant to the role. Evidence of practice within the four pillars of Advanced Practice; Clinical practice; Leadership and Management; Education-facilitation of learning; Research and Development.

- Knowledge of local, national, strategic and operational policy developments and an awareness of their implications.
- Willingness to undertake training required for the role.
- Hold a UK Driving Licence or equivalent and have access to a vehicle during working hours.

- Coaching / Teaching and Assessing qualification / experience.
- Non-Medical Independent & Supplementary Prescriber or willingness to undertake. Pharmacology and Prescribing Module at Masters Level
