Job summary

The Medicine Optimisation Clinical Lead Pharmacist role will be varied. The post holder will, on behalf of the BNSSG system NHS@Home service, lead the medicine optimisation and pharmacy workforce required to support the clinical service, alongside working as a senior clinical practitioner. The role will be split approximately 50:50 for the two functions.

You will work collaboratively with the Associate Director and Medical Director to develop and improve medicines management within the @Home pathways both in the acute hospital and community ensuring equitable access to medicines for patients. You will work closely with other pharmacy strategic leads as well as the clinical pharmacists in the team to lead and manage the clinical and governance components of the medicines optimisation pathways, ensuring policies and procedures are in place.

There are opportunities for a development post as an alternative to this full role with a clear plan to move to this role in its entirety on completion of training & competencies .

Main duties of the job

You will work closely with other pharmacy strategic leads as well as the clinical pharmacists in the team to lead and manage the clinical and governance components of the medicines optimisation pathways, ensuring policies and procedures are in place.

You will provide direct clinical care as part of the multidisciplinary team to:

    • Ensure co-ordinated care to patients
    • Reduce hospital re-admissions
    • Improve medicines safety
    • Improve medicines compliance

As part of our Advanced Practice team you will provide high level clinical assessment and interventions for patients in NHS@Home pathways demonstrating high level clinical reasoning and independent prescribing.

There will also be opportunities to engage in research and service improvement as part of the role.

Please note, we are able to offer the successful candidate between 30 to 37.5 hours per week. Your preference can be discussed upon interview, which we are aiming to hold on the week commencing 8th January 2024.
