About the Stroke programme across our region:
Across Bristol, North Somerset and South Gloucestershire,
health, care and voluntary sector partners are working together to improve
services for people affected by stroke. This includes work to improve the
prevention of stroke, as well as to provide the best possible stroke emergency
care, rehabilitation and care at home once individuals are discharged from
hospital. Our vision is to provide seamless, multidisciplinary care, so local
people have the best opportunity to survive and thrive after stroke.
This role is based in Sirona's ICSS at Home
team
ICSS at Home aims to reduce the length of time that people
are in hospital by providing high quality home-based rehabilitation, helping
individuals achieve their personal goals. We also provide advice and support to
reduce the likelihood of further strokes and help people live well as they
recover.
The care is provided
by a multi-disciplinary team of therapist, registered nurses and support
workers. Our team support people from the day they are discharged from hospital
or a rehabilitation unit. This includes personal care and meal preparation,
working side by side with voluntary sector keyworkers to support service users
in the best way possible.
Main duties of the job
As a Rehab Support
Worker you will be supporting registered health care professionals to deliver
rehabilitation programmes and assist with care and support of patients
referred to the service. You will also
provide non-clinical support to the Specialist Stroke Service.
You will be responsible
for carrying out rehabilitation plans as prescribed by registered therapists,
in accordance with instructions and recognised training received. You will be
required to be autonomous and use decisions making skills. You will provide
assistant to clinical staff in the assessment and treatment plans of patients
in the Specialist Stroke Service.
We have
a range of full-time and part-time opportunities available in our South Gloucestershire team. You
will be required to work a variety of shifts over a 7 day period between the
hours of 8am-8pm. A driving licence and access to your own transport is
required.
Enhanced rates for evenings and weekends is applied, plus a competitive 65p per mile mileage rate.We focus heavily on career development and there are lots of opportunities to undertake training and progress on to Trainee Nurse Associate or Assistant Practitioner courses. We have employees who started with us as Support Workers and are now Registered Nurses/Therapist all through our internal opportunities!

health and social care.

support in the monitoring, ordering and receipt of equipment into the team
peripheral store.
·
To
carry out general administrative and non-clinical work to contribute to the
functioning of the team such as photocopying, filing, liaison and telephone
calls.
·
Provide
and maintain a safe and comfortable environment all patients and carers,
seeking support from senior staff if the need arises.
·
Carry
out safe and effective manual handling procedures to support service users to
maintain safety throughout interventions.
·
To
document electronic case notes, which meet Sirona’s Standards for
documentation.
·
To
be flexible to meet the demands of the service as a whole across BNSSG, working
evenings, weekends and bank holidays where required.
·
Participate
in regular team meetings and attend other meetings as deemed necessary for
role.
