Job summary

An exciting opportunity has arisen for a band 6 heart failure specialist nurse to join our community-based service. We are looking for a dynamic nurse to join our well-established service that covers Bristol, North Somerset and South Gloucestershire (BNSSG)

Our community-based specialist service works to improve outcomes for patients living with heart failure. By working as part of a wider Multi-Disciplinary Team we strive to deliver best evidence care that is individualised for our patients. We work closely with our partners in Primary and Secondary Care, providing early treatment intervention and supporting the transition from hospital to long term management. The service also offers advice and guidance to Primary and Community Care.

You will see patients in community-based clinics and in their own homes and therefore a full UK driving licence is essential or ability to be able to travel independently to fulfil the role.

The services core hours are currently Monday Friday 0800-1700

There will be an element of working across BNSSG to meet service delivery, but we look to deliver clinical caseload elements of the service within geographical locations with bases currently within Bristol and North Somerset

The team is supported by a Consultant Cardiologist

Main duties of the job

The post holder will manage a caseload of patients with complex Heart Failure, using evidence-based practice and patient centred goals to assess, plan, implement and evaluate interventions.

To provide expert support, advice, and education to raise awareness of the needs of patients with Chronic Heart Failure

To undertake all aspects of clinical duties as an autonomous practitioner with access to advice and support from senior colleagues

The ideal candidate will have a strong team work ethic; excellent communication skills; a committed professional approach to working with both patients and the team; a strong sense of accountability, accompanied by an ability to work under pressure at times.

Qualifications

Essential

  • Current registration with NMC / HCPC or other relevant professional body of registration
  • Hold a Full UK Driving License and have access to a vehicle for work purposes.

Desirable

  • Glasgow Caledonian Heart failure course or related course or willingness to undertake.
  • Willingness to undertake training required for the role such as Independent Prescribing if not already completed or PADRAP/PACR
  • Physical Assessment and Diagnostic reasoning course or equivalent or willing to undertake
  • Teaching and assessing qualification

  • Previous/recent cardiology experience.

  • Demonstrate ECG interpretation skills.
  • A commitment to equality and diversity and the ability to personally demonstrate Sirona values
  • Able to demonstrate an understanding of the implications of cultural differences within the health care setting.

Desirable

  • Previous experience of working in a community/ primary care setting
  • Experience of teaching multidisciplinary groups
  • Experience of undertaking health promotion activities with patients and carers.
  • Experience and knowledge of principles and practice of palliative care
  • Knowledge of shared decision making and goal setting. Ability to implement this within patient care
  • Be able to undertake comprehensive specialist assessments of patients including those who may have complex presentations or multiple pathologies
