Job summary
The BNSSG NHS@Home service is a ground-breaking joint initiative by local NHS organisations that offers hospital-standard care and remote monitoring in an individuals home, providing an alternative to hospital admission, or helping them to return home promptly and safely following an inpatient stay. This is an exciting opportunity to join an emerging service and work collaboratively with other healthcare professionals to keep people comfortable, where they feel safe and can be with their loved ones.
As a team we are blending medical innovation with bespoke care, bridging the gap between hospital and home. A virtual ward is a safe and efficient alternative to NHS bedded care that is enabled by technology and home visits. This innovative approach enables us to deliver safe, high quality and personalised care to people at home where they would rather be.
We know that people recover faster and better in their own environments and our dedicated teams of trained staff endeavour to support this. A hybrid model of virtual, telephone and face to face consultations can be delivered as required, allowing patients to recover in their own home with continued clinical support.
The Hub team currently provides support for Respiratory, Frailty, Heart Failure and Home IV patients, with the desire to develop and expand further pathways
Main duties of the job
We
are looking to recruit a Band 3 Rehab Support Worker with relevant skills,
experience and qualifications working with service users on our NHS@Home
Pathways.
Key
responsibilities include:

undertaking a range of suitably delegated
clinical, care and therapy led activities within a competency framework
- Provide a safe, patient centred,
effective and evidence-based clinical activities in the community setting.
- Provides
support to other services where the need arises for contingency
Base location: John Milton Clinic, Henbury
Service hours: 8am-8pm, 7 days per week including bank holidays. Your
shifts will be allocated on a rota basis and will incorporate a variety of
shifts within the above hours

- Contribute to the holistic assessment of service users
- Deliver clinical care delegated by the registered healthcare professional, in accordance with an agreed person centred plan of care.
- To undertake and complete the appropriate training, supported by a robust competency framework, within the time allocated, in order to carry out the requirements of the role and a deliver a competent clinical service.
- Continuous maintenance of competencies following policy and avoiding gaps in service through lapsed competencies
- Work within your level of competence, only performing those tasks you have been trained to do and feel competent to carry out
- Escalate any changes in patient presentation or concerns you have to a more senior member of your team
- Assist clinical staff in the assessment and treatment of patients referred to the NHS@Home service. Following prescribed aspects of care, supervising patients with personal and domestic living activities, adaptions of mobility aids and the provision and fitting of basic aids.
- Take every opportunity to support and educate service users and their carers to promote independence, good health and personal safety.
- Provide and maintain a safe and comfortable environment for all patients and carers and seek further advice from trained staff if an emergency situation should arise
- To communicate effectively with patients and other stakeholders verbally, in writing and electronically using E-mail. This includes writing clinical records in line with the Sirona care & health Documentation Policy
- Adhere to organisational policies and Standard Operating Procedures related to area of work.
- Contribute to the movement and handling of individuals to maximise their comfort and safety.
- Undertake further training as required in order to develop extended skills and enhance patient care.
- Contribute to audit and data collection as required
- Identify risks and act accordingly to minimise risk to self, service users and colleagues in line with Organisational policy and procedures
- Clinical Skills such as clinical observations, venepuncture and supporting patients with monitoring equipment.
- Work flexibly to meet the service needs and undertake any other duties that may be reasonably requested to help the service run smoothly, this may include administration support duties when required
- Be able and willing to travel throughout the service delivery geographical area
