We also provide advice and support to reduce the likelihood of further strokes and help people live well as they recover.

The care is provided by a multi-disciplinary team of therapist, registered nurses and support workers. Our team support people from the day they are discharged from hospital or a rehabilitation unit. This includes personal care and meal preparation, working side by side with voluntary sector keyworkers to support service users in the best way possible.
