We have a rare and exciting opportunity to recruit Band 6
Senior Staff Nurse/Team Leader in Elton Ward, an 8 bedded inpatient
rehabilitation unit located within North Somerset Community Hospital.
Elton Ward provides care for patients on discharge from hospital supporting
home is best principles.
We are looking for a leader who has a genuine interest in
working with older people and delivering high standards of care and is
passionate about providing person centred rehabilitation care. If you think

As a Team Leader you will be responsible on a day to day
basis for coordinating shifts, providing care and support for patients who
present with a broad range of conditions including orthopaedics, frail elderly,
post-surgery and long term neurological conditions.

The post holder will be expected to be flexible and able to
work a shift pattern over a range shifts across a 7 day period, inclusive of
weekends and bank holidays.
Main duties of the job
The role will require you to:
- Support and work alongside the multidisciplinarythe team to deliver excellent care and work with the ward manager to
ensure that standards of care are maintained and to encourage your colleagues
to grow and develop.
- Lead a team to support an environment that provides our patients with the best
experience and treatment possible to help them attain the highest level of
personal independence prior to their discharge from our service
- Have an interest in improving the patient experience and demonstrate excellent
communication skills and develop effective working relationships with all
members of the multi-disciplinary team.
- Support the ward manager including e-rostering, reviewing and investigating incident
reports, undertaking personal development reviews, and mandatory monthly audits
to monitor quality care on the ward.
