Job summary

As part of our Learning Disability & Autism Hospital Liaison service, we are seeking a clinician (Nurse or AHP) who has a commitment to enabling positive outcome for people who have a learning disability and/or autistic, within an acute hospital setting.

We are a warm, welcoming and innovative team of specialist nurses whos primary aim is to ensure individuals who have learning disabilities and / or are autistic have equitable access to acute hospital provision and specialist healthcare. This is achieved through collaborative working with service users, carers, community colleagues and Hospital staff to provide reasonable adjustments to enable access to Hospital care, and by offering staff training. There is a wide variety in the needs of people we support and the situations we support with, making this an interesting role with varied days. You will also be required to have problem solving skills, be able to manage expectations and use your judgement.

Key objectives of the service are for individuals to receive the safe care they need, to provide reasonable adjustments, to provide timely intervention to facilitate access to outpatient appointments, support inpatient admissions and emergency department attendance and to support safe discharge and help avoid delayed discharge and to reduce the risk of avoidable readmission.

Main duties of the job

Your primary purpose will be to support the provision of the Learning Disability & Autism Liaison service in the Hospital.This includes:

- Receiving and responding to referrals, and prioritising needs.

- Assess and advise accordingly on the issues surrounding people with learning disabilities and /or Autistic patients who may have complex needs on their admission to hospital.

- Advise on the provision of reasonable adjustments

- Provide advice and guidance regarding Mental Capacity, Consent, Best Interest and Deprivation of Liberties to the service user, their family / carer and any professionals involved in decision making about their care.

- Working in partnership with the acute hospital trust and other healthcare providers within the Trust boundaries. Establish and maintain effective relationships with colleagues, other agencies, people who use the service and their families.

- Raise the profile of the health care needs of people with learning disability and Autism across the Trust, via training, campaigns and awareness raising. This can be in collaboration with experts by experience.

- Maintaining clinical records.

- You will contribute towards the development of the wider Learning Disability & Autism liaison services across BNSSG by working collaboratively with colleagues to meet the objectives of the services.
