Job summary
This is an exciting opportunity to join our Diabetes and Nutrition and Dietetic Service due to recent internal promotion.
We have a range of clinical pathways, including IBS and Nutrition Support working with service users with a variety of conditions such as stroke, neurological diseases and learning disabilities. As well as a large diabetes pathway, that includes the delivery of patient diabetes structured education collaboratively with the diabetes specialist nurses.The service works across a range of different settings including rehabilitation and stroke sub-acute rehabilitation units, residential care homes, service users own homes and in a community clinic environment. We provide dietetic support to the Early Supported Discharge stroke pathway as part of the new Integrated Community Stroke Service.
As a specialist service we work closely with other services especially the Adult Learning Disabilities Service and Tissue Viability Team, Respiratory Team and Bladder and Bowel Specialist Services, together with our community Integrated Network Teams and primary, secondary, and social care partners.
The successful applicant will have the opportunity to develop a plan that could work across all pathways or within specific areas.

Main duties of the job
Working as part of the Integrated Diabetes and Nutrition Service you will work with stakeholders to promote the highest possible quality of life, short and long term for the individual within the clinical pathway by:
- Delivering specialist, evidence-based community dietetic consultations which provide holistic assessment, support self-management strategies and result in an agreed patient centred care plan
- Acting as a source of specialist, evidence-based information, advice and support for colleagues and other health professionals regarding nutrition and advice for patients and carers
- Contributing to the development and review of evidence-based policies, protocols and pathways and embedding these in practice events including hard to reach communities
- Working collaboratively to enable knowledge and skills development, with the aim of supporting service users health needs and outcomes.
- Participating in audit, research activities, pathway and policy development, staff, and student supervision and training
- Delivering education and training on nutrition related topics to a range of healthcare professionals and patients
