Across Bristol, North
Somerset and South Gloucestershire, health, care and voluntary sector
partners are working together to improve services for people affected by
stroke. This includes work to improve the prevention of stroke, as well as to
provide the best possible stroke emergency care, rehabilitation and care at
home once individuals are discharged from hospital. Our vision is to provide
seamless, multidisciplinary care, so local people have the best opportunity
to survive and thrive after stroke.
This role is based in
Sirona's ICSS at Home team
ICSS at Home aims to
reduce the length of time that people are in hospital by providing high
quality home-based rehabilitation, helping individuals achieve their personal
goals. We also provide advice and support to reduce the likelihood of further
strokes and help people live well as they recover.
The care is provided
by a multi-disciplinary team of therapist, registered nurses and support
workers. Our team support people from the day they are discharged from
hospital or a rehabilitation unit. This includes personal care and meal
preparation, working side by side with voluntary sector keyworkers to support
service users in the best way possible.
Main duties of the job
As a Band 5 Occupational Therapist in our integrated Community Stroke Service (ICSS) you will be providing safe, patient centred, effective and evidence-based Occupational Therapy service in the community setting. You will deliver both acute and long term therapy and clinical activities, supported by a robust competency framework to adults within their own home and other settings.
You will be part of an integrated multi-disciplinary team (MDT), which includes Nurses, Physiotherapists, Speech & Language Therapists, Support workers and other Healthcare Professionals. Support and training will be provided from your wider team, helping you to specialise in the area of Stroke and enable your career development.
Our clinical teams are required to work a range of shifts over 7 days between 8am-4pm. A driving licence is required.

With the support of senior staff be responsible for the
initial holistic assessment, implementation and evaluation of programmes of
care for patients following a stroke

Use clinical reasoning skills to assess, plan, implement and
evaluate, patient- centred intervention programmes incorporating physical,
psychological, social, leisure an employment needs, using an evidence based
practice approach, promoting high quality and harm free care.

Collaborate and negotiate with service users, carers and
family in identifying appropriate and achievable goals as part of the overall
care plan, which will maximise quality of life and independence.
·
Facilitate appropriate referrals to other services or
investigations based on your assessment to ensure holistic patient care
·
Work within a multi-disciplinary team, to provide integrated
skills and sharing knowledge providing holistic and specialist skills supported
by a robust competency framework

To apply theoretical and working knowledge of medical,
neurological, surgical, psychological, trauma and terminal conditions, then
work with the implications of these on the quality of life of patients and
carers.

To use occupational; therapy skills, techniques in functional
and standardised assessment & interventions as an autonomous practitioner
·
Create person centred, goal based treatment plans using
outcome based approach which can be delegated and supported by Stroke
colleagues.

To develop additional competencies within the requirements of
the multi-disciplinary approach to the Stroke Service
·
To assess and arrange the provision of equipment and/or
adaptations from appropriate agencies ensuring this is provided in good working
order and fitted correctly; reporting any issues appropriately
·
Supervise members of the team including peers, students and
unregistered workforce in occupational therapy competencies
