Job summary
An exciting
opportunity has arisen for a dynamic nurse to join the Intermediate Care Team
in First Community Health and Care.
The Intermediate
Care Team (ICT) are an admission avoidance and facilitating discharge service,
who provide assessment, short-term care and rehabilitation in the patients
home. You will complete comprehensive assessments in the patients home and
undertake treatments including IV medications, wound assessment, and
management. You will work closely with the multi-disciplinary team to promote independence of patients to ensure they can remain in their own
home.
The candidate will
be able to adapt to change and work in a busy environment. Excellent communication skills are essential
to enable liaison with all our health and social care partners in the management of patients discharge from hospital and assessment in the
community.
Main duties of the job
You will also provide professional leadership for designated staff and
will have a role in the leadership and line management of the wider multidisciplinary
team.
Due to the nature of the role it is essential that applicants are able
to drive, have a car suitable for business purposes and have a clean driving
licence.

The service runs
from 8am-8pm 7 days a week, 365 days a year, the post holder will be expected
to work on a rota basis. There are opportunities for both full time and part
