Job summary

The Urgent Community Response(UCR) Team aspires to provide a 24-hour/7-day responsive NHS community service. There is the flexibility within the multidisciplinary team to work across days, nights and to do internal rotation and you are able to work autonomously, managing patient assessments within the specialty whilst working as part of the larger multidisciplinary team, delivering individualised and personalised direct patient care to patients across Northwest Surrey in conjunction with the wider Integrated Care System. The teams are commissioned to reflect the needs of the local community. Service aims include urgent case management and hospital admission avoidance, where the focus of the role is to lead the identification and clinical assessment of patients who will benefit from advanced complex hospital discharge care, or urgent responsive admission avoidance using the Virtual Ward if required, with care provided in their own home by the multi-disciplinary team.

Main duties of the job

  • A To work as a member of the multi-disciplinary UCR team.
  • To undertake assessments, clinical diagnosis and treatment of patients including telephone triage, multi-factorial falls assessments, and patients who may require crisis management and/or rehabilitation interventions to prevent acute hospital admissions and to support complex hospital discharges. These patients often have complex presentations.
  • To provide a fast reactive service for patients with decompensated frailty and ensure rapid delivery of treatment and care planning to support acute hospital admission avoidance where appropriate with a focus on the 9 Common Critical Conditions
  • Work as an autonomous practitioner the post holder will undertake a complex and varied clinical workload and be an expert in rehabilitation within the home and other community settings as required.
  • The post holder will clinically manage and supervise Nurses, senior community rehab assistants, and students and support other clinical colleagues in the team as required. This will include supporting learning and other development activities.
  • The post holder will take a lead role in planning, coordinating, delivering and evaluating the service on a day-to-day basis, whilst working closely together with other internal and external stakeholders.

Clinical

Demonstrate appropriate expert practice and highly specialist advice in the assessment, planning implementation and evaluation of patient care. This includes history taking, physical examination and the requesting of agreed diagnostic procedures/clinical investigations.

Responsible for the correct administration of prescribed medication.

Promote and deliver high standards of care at all times, which utilises researchbased evidence, and is audited and improved accordingly. Act as a role model, demonstrating high standards of care and providing clinical leadership in the specialist areas.

Establish, develop and maintain effective working relationship between clinicians, practitioners and managers within the specialty, across the Trust and (ASPH) Ashford & St Peters Foundation Trust.

Actively participate in local, national and international networking to maintain and develop expert level of knowledge in the specialty.

Provide a seamless, high quality service from referral through to assessment, diagnosis, treatment and review.

Develop and evaluate assessment tools that will ensure an appropriate level of nursing intervention so that patients who present with the most complex needs receive the most appropriate specialist care.

Contribute to CSH Surreys clinical supervision programme for staff.
