Job responsibilities

Purpose:To work as a community rehabilitation assistant, as part of the Community Rehabilitation Team, aiding rehabilitation, within agreed level of competency, of patients within a designated location e.g. a community setting / home environment.

To work at times, independently, and as part of a team managing a delegated caseload, under the guidance and supervision of a qualified professional e.g. physiotherapists, occupational therapists, within set parameters, providing treatment regimes as prescribed, monitored and changed by qualified staff. To work towards achieving a high quality and effective Community Rehabilitation service. To provide administrative support to colleagues and services users.

Key Relationships include:Physiotherapists and Occupational Therapists within the Community Rehabilitation Team as well as collaborative working with Urgent Care Response / Rapid Response Team, Matrons, Allied Health Professionals, GP Practices, Professional and Technical staff, Inpatient Services and Discharge Teams, Social care services, Administrative and Clerical Staff.

Principal duties to include:

Following appropriate induction, supervision and competency-based training; you will be able to work alone with patients in their own home or an environment appropriate to their needs, providing prescribed rehabilitation programmes of care.

To be able to monitor progress with appropriate outcome measures

Has a thorough understanding of rehabilitation provided by the Community Rehabilitation Team and is able to communicate this effectively to other members of staff when required.

To provide personalised therapeutic input as stated in patients individual intervention plans (which are set by a health care professional) whilst encouraging the patient to remain as independent as possible.

To fulfil any specific tasks (within level of competency) as requested by the therapists

To carry out programmes devised by therapists using a range of techniques which the post holder has been deemed competent to do.

Responsible for informing, discussing and changing visits with clients as agreed by the multi-disciplinary team.

To be able to prioritise own caseload

To assist qualified staff with monitoring the outcomes of therapeutic interventions.

To understand and work within CSH and teams Lone Working Policy and local procedures.

Responsible for contacting clients if delayed for any unforeseeable reason.

To have an understanding of local administrative processes and support the team with these tasks in team Administrators absence.

To communicate effectively appropriate information in an understandable format to patients, carers and other staff.

Understand patients right to choice, privacy and dignity and the need to treat patients as individuals.

Responsible for reading and adhering to patients intervention plan prior to commencing activities.

Responsible for time management and spending appropriate time on each visit. To use effective communication skills to gain co-operation with treatment where there may be significant barriers to understanding such as sight, hearing and language barriers.

Responsible for the delivery of care and rehabilitation without prejudice maintaining a professional manner in sometimes adverse conditions in patients home environment.

Responsible for maintaining own competencies in all matters of patient care and rehabilitation as appropriate for example; personal care, infection control, the use of protective clothing, waste disposal, health and safety, safe usage of equipment, patient transfers, correct walking gait, cognitive and functional activities. Inform line manager or supervisor immediately if you do not feel competent to carry out task.

Can demonstrate an awareness and understanding of individuals, physical conditions / symptoms, emotional, psychological, social and spiritual and cultural needs.

Has an understanding of the additional services available in the community and when and how to refer to these services, for example Meals on Wheels, Red Cross, Citizen Advice bureau Age Concern other voluntary services.

Is able to identify changes in patients condition and within individuals expected level of competence acts accordingly, and in a timely fashion. If appropriate seeks advice from Team Leader or other professional before calling emergency services.

Following training and deemed competent, use any aids and equipment (e.g. walking aids, chair raisers and commodes) as required ensuring appropriate sizing and education of patients in correct use of appliance/s. Report defects in equipment to appropriate member of team.

Responsibility for monitoring peripheral equipment store (in the absence of other staff member) and ensuring correct procedure is followed when equipment is delivered to or removed from the store.

To collect and deliver equipment for clients in line with safe working practices, health and safety and infection controls policy and procedures.

Understands the need for confidentiality and the Data Protection Act when recording patients personal and sensitive information. Maintains accurate, comprehensive, legally compliant and up to date patient notes in line with professional standards and local trust policies. To adhere to set clinical protocols as required. Aware and responsible in helping to maintain safe working environment. Has a working knowledge of CSH Surrey Health and Safety policy.

Responsible for representing the Service in all activities, including introducing visitors to the service and ensuring patients receive the highest standard of care within given resources.

To understand the policies and procedures of the CSH Surrey with emphasis on working with patients and colleagues in a community setting.

To be involved in service development projects.

To be aware of possible risks in patients home environment prior to undertaking individual patient care plans as directed by the team professionals.

Performance:

The post holder has a responsibility to participate in personal and performance review and to undertake identified training needs and development. The post holder is required to demonstrate on-going continuing personal development.

To undertake clinical supervision on a regular basis

To attend mandatory / statutory / In service and any other training programmes as Identified in your personal development plan.

To participate in regular 1:1 with your line manager or supervisor.
