Job summary

We are looking for an enthusiastic and motivated Band 7 Physiotherapist Team Lead to join our highly regarded Urgent Community Response (UCR) team based at Woking Community Hospital, Woking.

As a key partner in the delivery of urgent responsive services to support the frail population in hospital admission avoidance, the service is keen to offer opportunities and to support the development of team members to extend their skills and broaden their scope of practice where appropriate.

Although expected to work autonomously you will not be alone. The team includes Occupational Therapists, Nurses, Paramedics, Community Rehabilitation Assistants and Administrators in addition to the Physiotherapists; and we work closely with colleagues from the Frailty Hubs, GP surgeries, adult social care and other community services.

We provide a 7-day, 24-hour urgent response NHS community service to patients across Northwest Surrey.

Main duties of the job

Working as a member of the multi-disciplinary team you will undertake holistic assessments, clinical diagnosis, and treatment of patients in their own home environment, who may require crisis management and or rehabilitation interventions to prevent acute hospital admissions, to support complex hospital discharges and maximise their independence.

The role will include the triage and face to face assessment of patients on the Urgent Community Response and Virtual Ward pathways, and often the assessment and provision of equipment to support safe mobility and activities of daily living. As a result of your assessment, you may also arrange short term support provided by our own Senior Community Rehab Assistants, and liaise with colleagues in local GP practices, locality frailty hubs, adult social care teams, and other community services.

Essential

  • Minimum of 5 years post registration experience working in the NHS in hospitals and community
  • Extensive clinical experience working with older people, stroke, neurological disorders, rehabilitation
  • Experience of clinical supervision and management of junior staff and other staff
  • Experience of supervision of students.
  • Experience of lone working and decision making. Knowledge of clinical governance
  • Knowledge of relevant NSFs, clinical guidelines and current NHS directives
  • Experience of leading on changes in practices and service developments
  • Experience of working in a multidisciplinary team and contributing to team decision

Qualifications

  • Registered with HCPC Physiotherapist Diploma / Degree
  • Post graduate specialist training and relevant courses

  • Appropriate training at MSc level or equivalent experiential development
