Job summary

We are looking for an enthusiastic and motivated Band 7 Occupational Therapist Team Lead to join our highly regarded Urgent Community Response (UCR) team based at Woking Community Hospital, Woking.

As a key partner in the delivery of urgent responsive services to support the frail population in hospital admission avoidance, the service is keen to offer opportunities and to support the development of team members to extend their skills and broaden their scope of practice where appropriate.

Although expected to work autonomously you will not be alone. The team includes Physiotherapists, Nurses, Paramedics, Community Rehabilitation Assistants and Administrators in addition to the Occupational Therapists; and we work closely with colleagues from the Frailty Hubs, GP surgeries, adult social care and other community services.

We provide a 7-day, 24-hour urgent response NHS community service to patients across Northwest Surrey.

Main duties of the job

Working as a member of the multi-disciplinary Urgent Community Response (UCR) team you will undertake assessments, clinical diagnosis and treatment of patients who may require management and/or rehabilitation interventions in their own environment to prevent acute hospital admissions and to support complex hospital discharges.

Although expected to work autonomously you will not be alone. The team includes Physiotherapists, Nurses, paramedics and Community Rehabilitation Assistants in addition to the Occupational Therapists; and work closely with colleagues from GP surgeries, locality health hubs, adult social care and other community services.

The qualities we are looking for in successful applicants are:

Experience working as a senior clinician in admission avoidance services.

Excellent communication and time management skills.

Flexible approach to work with the ability to work across a seven-day service.

Ability to work both independently and as part of the wider multidisciplinary team.

Willingness to undertake ongoing professional development and a commitment to providing high quality, safe and effective care that meets the needs of patients.

Car owner with appropriate license to drive in the UK for business purposes

This role provides a unique opportunity for an Occupational Therapist (OT) to work as part of an integrated, community-based Urgent Community Response (UCR) service in North West Surrey. The post holder will work as part of a multi-disciplinary team to deliver holistic assessments, interventions, and ongoing support to adults with a variety of complex needs, ensuring safe, high-quality care is provided within the community setting. The OT will provide a rapid assessment and short-term interventions to prevent hospital admissions and enable patients to live independently for as long as possible. The post holder will be responsible for delivering specialist Occupational Therapy interventions within the UCR team, including conducting assessments, providing therapy, and supporting the rehabilitation of individuals. They will also provide leadership to junior staff and promote the integration of OT into the multidisciplinary team.
