Job summary

This is an exciting challenge for a motivated and innovative Health Visitor to join us as a Community Practice Assessor, in our experienced, skill mixed 0-19 team based in Leatherhead, Dorking (Mole Valley), Banstead and Tattenham.

The ideal candidate for our team would be a clinical practitioner with leadership and excellent interpersonal and communication skills. We look for individuals with a strong approach to teamwork, those who demonstrate initiative, a good understanding of children, young people and their families and with the ability to provide clinical and professional leadership at an operational level in order to provide expert high quality, integrated and well-coordinated services to Children and Young People. Engaging and empowering the professional team to provide excellent, outcome driven, evidence-based practice in partnership with families, the local community and stakeholders.

Our teams offer a strong leadership structure, friendly working skill mix team and admin support. We also have good working relationships with partner agencies such as Midwifery, Social Care and Surrey Safeguarding Board.

You will be supported by our Clinical Service Managers, work alongside Clinical Team Leaders and Safeguarding Team to provide excellent professional practice and to further develop your skills and knowledge.

Job description

Job responsibilities

Principal duties to include:

Implement key aspects of evidence-based practice.

Demonstrate clinical expertise and act as a professional role model to all staff, both internal and external to CSH Surrey, working as part of a team and taking the lead to develop services in line with the needs of the client.

Work as part of the multi-disciplinary/multi-agency team to provide a comprehensive and co-ordinated range of support which improves or maintains the health, independence and quality of life of children, young people and their families

Apply highly specialist knowledge and skills which are underpinned by advanced theoretical and practical knowledge and experience. Autonomously manage a caseload of children that enables sufficient time and practice experience to support SCPHN student, this may include long arming students in an adjacent borough.

Participate in the orientation and induction of new staff and promote good interpersonal relationships within the team

Contribute to the pre and post registration learning opportunities made available to students within CFHS, which may also include alternative practice placements or additional field of practice opportunities for SCPHNS.

Complete annual team educational placement audits Plan learning in conjunction with the university, student and other learners recognising individuals particular needs, interests and learning styles

To hold responsibility for the assessment of clinical practice for SCPHN students within the community

To facilitate and participate in a preceptor programme for newly qualified specialist practitioners

Working with the clinical team lead and clinical service manager contribute to the clinical management of the multi-professional team, and lead in the evaluation, planning and development of service pathways in relation to effectiveness and best practice. This will include line management, training, supervision and expert advice to other professionals and parents.

Work as an integral part of the children and families multi-disciplinary team, and as a specialist resource to other professionals and agencies. Develop and participate in the team based continuous professional development (CPD) activities and provide training for less experienced members of the team.

Provide formal and informal training and support to groups of health and education professionals and support workers to promote and enhance patient care.

Lead on research to ensure and support the development of evidence based practice in your specialism.

Initiate and maintain contacts with local and national clinical interest groups appropriate to the clinical field and to share and disseminate knowledge with colleagues working within the directorate.

Motivate team members to develop innovative ways to practice creating an environment in which co-owners accept responsibility, authority and accountability for their actions.

Contribute to the delivery and provide specialist knowledge and advice to influence the CFHS strategic agenda.

Actively seek, understand and apply national guidelines/legislation relating to health and social care in the provision of childrens services.

Demonstrate an advanced in depth understanding of clinical governance and clinical risk

Propose &/or contribute to the development of policies & procedures for the Children and Families Business unit.

Work in partnership with GP practices, nurseries, childrens centres, schools and other agencies within area of responsibility in order to meet local and national objectives.

Provide support to Clinical Team Leader in disseminating learning and new practice developments to borough teams

Participate in the development of service for children and young people across CFHS

Support the teams through periods of change; ensuring skills of the individuals meets the needs and expectations of both the profession and the modernisation agenda.

Provide support for the clinical service manager and other leaders as required.

Work with key personnel in health and other agencies to address and/or achieve agreed local and government policies and targets.

Identify the individual potential in all co-owners within the team and support them through effective annual professional development review processes.
