Job responsibilities
Purpose
To support the nursing team with the provision of high-quality wound management, leg ulcer management and prevention, ensuring all care is holistic and patient focused. To provide clinical support as required to help ensure the efficient and effective running of the clinical area. To utilise excellent communication skills which promotes the reputation of the Complex Wound Clinics. Has a flexible approach, enabling the smooth running of the Complex Wound Clinics across all localities according to need.
Principal duties to include:
Clinical
Undertakes holistic patient assessment within scope of practice, adapting care plans within prescribed parameters
Escalate any concerns to Registered Nurse Team
Establish and maintain good communication with patients, relatives and carers
Work in partnership with the multidisciplinary team in the provision of holistic patient care
Contributes to own personal development
Assist in maintaining own and others health, safety and security within the workplace
Contributes ideas for service and quality improvement
Works flexibly to meet the demands of the service
Communication skills
Communicates with people clearly in a manner, and at a level of understanding appropriate to their abilities, preferences, and beliefs
Actively listens and responds to the needs of patients, relatives, and carers
Provides assistance to patients, relatives and carers, utilising persuasive skills, reassurance, tact and empathy as required
Communicates information only to those people who have a right and a need to know, respecting confidentiality, in line with The Data Protection Act.
Analytical and judgemental skills
Actively assess peoples health, safety and wellbeing whilst delivering wound care
Recognise when there is a change in an individuals health and well-being, which requires escalation to a member of the registered nursing team, GP or other health or social care professional
Planning and organizationalskills
Delivers day to day duties according to plan
Prioritizes the care requirements for identified patients
Assists in the organizationof work as required
Patient / client care
Competently undertake tasks that have been delegated
Record information accurately and pass it to the relevant people in the team in a timely manner
Identify and report any changes that might affect the patients condition
Obtain agreement from patient/carers before commencing any care related tasks
Respect the individuals privacy, dignity, and beliefs.
Prepare and support the patient appropriately during any activity that is to be undertaken
To promptly alert the team where there are any changes in the patients health and wellbeing or any possible risks

To record data accurately using the agreed systems i.e. EMIS, paper records, electronic records

To participate in audit/benchmarking within the clinical area supporting the introduction of a change in practice if indicated

Works to established protocols, practices, and procedures.
Work is supervised within the acute setting. On occasion, work is not always supervised but support is available

Health and Safety
The post holder will be required to observe local Health and Safety arrangements and take reasonable care of him/herself and persons that may be affected by his/her work ensuring compliance with the requirements of the Health and Safety at Work Act (1974).
Health and Safety
- To follow and promote safe working practices and to comply at all times with the Health and Safety at Work Act 1974, and Manual Handling Operations Regulations 1992, and CSH policies and procedures
- To assist in the regular monitoring and maintenance of equipment in accordance with health and Safety regulations.
- To act immediately on safety notices, hazard warning notices and any other notifications in relation to equipment used/prescribed by the Occupational Therapy Service.
- To take reasonable care of the health and safety of yourself and other persons who may be affected by your acts or omissions at work and to co-operate with CSH to ensure that statutory and departmental regulations are adhered to.
- To report all clinical and non-clinical incidents or near misses promptly and when required to co-operate with any investigations undertaken.
Security
- It is a condition of employment that identification badges be worn at all times.
- All co-owners have a responsibility for security and the proper care of property. In accordance with standing financial instructions all managers have a particular responsibility for security and loss prevention arrangements in their areas of responsibility.
Continuous Improvement
The organisation has adopted a strategy for Continuous Improvement and all members of staff employed by the organisation are expected to play an active role in development and improving services to the benefit of service users. In-service training and development programmes are provided including the NVQ process.
Clinical supervision
Clinical Supervision is an ongoing process between clinical supervisor and clinical supervisee which aims to develop and sustain a high level of quality patient care by reflecting on clinical practice and provides encouragement for personal and professional growth. All Clinicians are expected to undertake clinical supervision in line with CSH Surrey policy
Registered Health Professional
All co-owners who are members of a professional body must comply with standards of professional practice/conduct. It is the post holders responsibility to ensure they are both familiar with and adhere to these requirements and maintain their professional membership to the relevant body.
