Job summary
An
exciting opportunity has arisen for a Clinical Case Manager in the Childrens
Continuing Care team, based at the
Jarvis Centre in Guildford.
Your
role will be to provide a consistently high standard of professional nursing
case management resulting in quality health care provision to children and
young people at home, in education settings and in the community. You will
enjoy carrying a clinical caseload and be autonomous in your diary management.
As
a valued member of the team, you will support the delivery of the service and embrace
transformation.
The
hours worked can be flexible, (to meet the needs of the service) covering
Monday to Friday, with a regular commitment to the evening/weekend staffing
on-call.
CSH

Main duties of the job
The purpose of the role is:
To provide a consistently high standard of professional
nursing and respite care to children the caseload at home, education and in the
community. To act as a mentor and line manage for Band 3 and 4 Childrens
Community Carers and Band 5 Staff Nurses.

Job responsibilities
- Responsible
for leading on the clinical care of the child, ensuring the clinical care
planning, equipment provision and care package is delivered effectively and
efficiently and updated appropriately, whilst ensuring children and families are
involved in the delivery and planning of their care
- Maintaining
professional working relationships with the child, parents and families of the
children on the caseload, communicating sensitively and effectively
- Maintaining
close links with community and both local and tertiary hospital based services,
keeping the lead clinicians clinically updated as appropriate.
- Communicates
effectively information including highly complex information with a wide range
of people, some of whom may have barriers to communication. This includes, children, families, carers and
other professionals working across Health, Social Services, Education and the
Voluntary Sector.
- To
promote safe clinical discharge from hospital for children with complex health
needs, liaising with health professionals, commissioners, educational
establishments, voluntary agencies and social services as required ensuring a
high standard of care is able to be delivered on full discharge of the child
- Effectively
works in partnership with social care and education when designing the agreed
care package.
- Responsible
for the planning, providing and delivery of high quality competency based
training and reassessment of competencies for Childrens Community Carers and
Registered Nurses and other agencies who work with the caseload. Evaluating the
training and supporting its development.

for providing training to school and nursery staff identified to facilitate the
childs journey into educational establishments
- Participate
in the Clinical On Call service for the caseload across a 7 day rota
- To work with the Clinical Team Lead, Commissioners
and Service Development Manager to co-ordinate discharge from hospital of
children who meet the criteria for NHS funded continuing care, liaising with
health professionals, voluntary agencies, educational establishments and social
services to ensure a high standard of care is delivered.
- To deal sensitively with families, carers and
colleagues when providing palliative care/ end of life care to children on the caseload
- Responsible
for raising with the team any matters affecting the service provided to the
child and their family or carer.
- Responsible
for communicating any concerns relating to the childs safety, and well being
through the appropriate routes and alerting the relevant staff and authorities

Making
decisions about the allocation of staff prioritizing the caseload clinically
and allocating the available resource accordingly which can often mean making
difficult decisions about who receives cover
- Participates
in the multidisciplinary development of service.
- Required
to undertake physical examination of the child/young person which may
include listening to the heart and lungs, catherisation, change of
tracheostomy breathing tubes, replacement of Percutaneous Endoscopic
Gastrostomy (PEG) feeding tubes, gastrostomy feed tube etc.
- Required
to have developed skills in the use of various equipment that requires
accuracy within a narrow margin for error e.g. syringe drivers,
ventilators, saturation monitoring, enteral/nasogatric/nasojejunal feeding
pumps etc.
- Driving
to visits and meetings; travel on the train to the Tertiary Hospitals in
London, to attend meetings, or training
- Following medicines management training
complies with the safe administration of medicines, assess Bands 3, 4 and
5 staff as competent to administer medicines. Produces a MAR chart
accurately according to the Medicines Management Policy and Production of
a MAR chart Standard Operating Procedure
- Maintain a high
level of clinical skill e.g. care and use of central lines, enteral
feeding devices, catheterisation, management of long-term ventilation,
tracheostomy care, wound care, stoma care, administration of specialist
infusions within the community setting and end of life care.

on the safe use of equipment to all Bands of staff and external agencies to a
high standard demonstrating in depth knowledge and understanding of the
