Main duties of the job

As a Band 6 CHC Nurse, your core duties will be central to delivering our service. You will be responsible for conducting comprehensive face-to-face assessments for individuals with complex health needs, applying the National Framework for Continuing Healthcare to determine eligibility. A key part of your role will be the development, implementation, and regular review of holistic, person-centred care plans in collaboration with patients, their families, and multi-disciplinary teams. You will act as a vital link, coordinating with care providers and other health and social care professionals to ensure the seamless and safe delivery of care packages. Your clinical expertise and judgement will be essential in monitoring care quality, managing risks, and championing the best interests of those we serve.

Job responsibilities

To work as a skilled and autonomous practitioner within the All Age Continuing Care (CHC) service. The post holder will be responsible for the assessment, commissioning, and review of care packages for individuals with complex, ongoing healthcare needs, ensuring the effective application of the National Framework for Continuing Healthcare and adherence to all relevant clinical guidelines and legislation.

2. Main Duties and Responsibilities

Clinical Practice & Assessment:

  • To undertake comprehensive, face-to-face assessments for individuals who may have a primary health need, utilising the Decision Support Tool (DST) as part of a multi-disciplinary team.

  • To apply a robust understanding of the National Framework for Continuing Healthcare and Funded Nursing Care (FNC) to ensure consistent, legally compliant eligibility decisions.

  • To conduct risk assessments and contribute to the management of complex cases, including those involving mental health, learning disabilities, and physical health needs.

  • To demonstrate high-level clinical reasoning and decision-making skills to determine appropriate care pathways.

Care Planning & Coordination:

  • To develop, implement, and evaluate highly personalised, evidence-based care plans in partnership with the individual, their family, and carers.

  • To commission and procure care services from a range of providers, ensuring value for money and that care packages meet the identified health needs.

  • To act as the key liaison between the ICB, care providers, social services, and other health professionals to ensure seamless, integrated care.

  • To chair and actively contribute to multi-disciplinary meetings regarding patient care.

Review & Monitoring:
