Main duties of the job

As a Band 7 Advanced Practitioner, you will be the lead expert for a complex caseload within our All Age Continuing Care service. Your core duties will involve chairing Multi-Disciplinary Team meetings to assess eligibility using the Decision Support Tool, and designing, commissioning, and reviewing personalised care packages. You will provide professional supervision and guidance to junior team members, ensuring the highest standards of practice. A key aspect of the role involves safeguarding our most vulnerable patients, which includes applying your knowledge of the Mental Capacity Act and Deprivation of Liberty Safeguards. You will act as a vital link between patients, families, and a wide network of health and social care providers across mid and south Essex to ensure seamless, person-centred care.

Job responsibilities

To provide expert leadership and management within the All Age Continuing Care (AACC) service. The post-holder will be responsible for the comprehensive assessment, coordination, and case management of individuals with complex health needs, leading on the determination of eligibility for NHS Continuing Healthcare (CHC) and the commissioning of bespoke care packages. The role requires advanced clinical knowledge, leadership skills, and a robust understanding of the legal frameworks surrounding complex care.

Professional and Team Supervision:

Care Planning and Commissioning:

  • Develop, implement, and regularly review highly specialist, personalised care and support plans for individuals eligible for CHC.

  • Lead on commissioning tailored packages of care, ensuring they are clinically effective, cost-efficient, and meet the individuals assessed needs.

  • Manage the financial aspects of each case within a delegated budget.

Safeguarding and Legal Compliance:
