Main duties of the job
As a Band 6 Multidisciplinary Pathway Worker, your primary focus will be on ensuring timely and safe care for young people. You will be the first point of contact, managing the duty line to triage needs and provide immediate support. A key part of your role involves conducting initial assessments to understand complex presentations and determine the best clinical pathway.
You will play a critical role in safeguarding by proactively reviewing the cases of those on the waiting list, conducting clinical risk assessments, and creating robust safety plans to manage any potential harm. Finally, you will collaborate closely with the team, bringing your assessments to MDT meetings to help formulate and coordinate effective, collective care plans.
