Main duties of the job

As a Mental Health Practitioner with our team, your core duties will be dynamic and patient-focused. A key part of your role involves delivering evidence-based interventions, regularly reviewing progress, and coordinating effectively with other healthcare professionals to ensure seamless support. Your clinical expertise will be crucial in conducting thorough risk assessments and developing robust management plans to safeguard service users in the community. This is a community-based position, requiring travel to meet service users in their own homes and local settings.

Job responsibilities

To manage a defined caseload of service users with a range of complex mental health problems within the community setting. The post holder will be responsible for the assessment, planning, implementation, and evaluation of care programmes, working autonomously and as a key member of the multidisciplinary team. The role requires a high standard of clinical expertise, risk management, and a commitment to recovery-focused practice.

2. Main Duties and Responsibilities

Clinical Practice:

  • To undertake comprehensive assessments of service users, including those with complex mental health needs, to determine their health, social care, and risk profile.

  • To develop, implement, and evaluate evidence-based, person-centred care plans and treatment pathways in collaboration with service users, their families/carers, and the multidisciplinary team.

  • To provide a range of specialist therapeutic interventions appropriate to the service users needs.

  • To regularly review and monitor care plans, making adjustments as necessary to support recovery and well-being.

Caseload Management:

  • To efficiently manage and prioritise a personal caseload, ensuring timely and effective coordination of care.

  • To maintain accurate, confidential, and up-to-date clinical records in accordance with professional and service standards.

Risk Assessment and Management:

  • To conduct systematic risk assessments, formulating and implementing robust risk management plans to safeguard service users and others.

  • To proactively manage crisis situations, providing support and intervention as required.

Multidisciplinary Team Working:

  • To work cohesively as part of a multidisciplinary team (MDT), contributing to team decisions, case discussions, and clinical reviews.
