Main duties of the job

This is a fantastic opportunity for a senior nursing professional to play a vital role in our Continuing Healthcare team, ensuring the delivery of high-quality, patient-centred assessments in line with the National Framework.

About the Role:

As a CHC Nurse Assessor, you will be responsible for facilitating multi-disciplinary assessments and patient reviews for NHS Continuing Healthcare. You will be a key clinical professional, ensuring that complex patient needs are thoroughly evaluated and that decisions are made in accordance with standard operating procedures and national guidance.

This is a patient-facing role requiring daily travel across the Northamptonshire area to conduct face-to-face assessments.

Key Responsibilities:

  • Manage a complex caseload, ensuring services are delivered to maximise patient safety and adhere to all guidance.

The Ideal Candidate Will Have:

  • A registered nursing qualification (RNLD, RMN, or RGN) with current NMC registration.

  • Substantial and proven experienceworking within NHS Continuing Healthcare, including conducting complex assessments and interpreting evidence.

  • Expert knowledge of the NHS CHC National Framework, safeguarding processes, and the Mental Capacity Act.

  • The ability to analyse complex information, make difficult, contentious decisions, and recommend appropriate courses of action.

  • Excellent interpersonal and communication skills, with the ability to influence and persuade in challenging situations.

  • A professional, calm, and efficient manner, with strong organisational and problem-solving skills.

  • The ability to work autonomously, prioritise a demanding workload, and meet tight deadlines.

  • A full driving licence and access to a vehicle for daily travel across Northamptonshire.
