Main duties of the job

As a Band 7 Pharmacist with us, your expertise will be central to our patient care pathway. You will clinically verify prescriptions, provide specialist advice to our multidisciplinary team on ward rounds, and counsel patients on their medication to support recovery and successful discharge. Your role is crucial in safeguarding the safe, effective, and evidence-based use of medicines across our inpatient services.

Job description

Job responsibilities

To provide a high-quality, patient-focused clinical pharmacy service as part of a multidisciplinary team (MDT). The post holder will be responsible for the clinical verification of prescriptions, providing expert advice on medicines optimisation, and supporting the safe and timely discharge of patients within the inpatient mental health services.

2. Main Responsibilities

Clinical Services

  • To perform clinical verification of prescribed medication, ensuring it is safe, clinically appropriate, and cost-effective in accordance with local and national guidelines.

  • To provide specialist advice on pharmaceutical care to doctors, nurses, and other healthcare professionals.

  • To counsel patients, and their carers where appropriate, on all aspects of their medicine, including side effects, adherence, and the use of new medicines.

  • To support the dispensary with clinically screened supplies for discharge and patient leave.

  • To participate in ward rounds and multidisciplinary team meetings as required, contributing to treatment planning.

Medicines Safety and Governance

  • To practice in accordance with professional standards and all legal requirements as set out by the General Pharmaceutical Council (GPhC).

  • To adhere to all local policies, procedures, and clinical guidelines.

  • To identify and report any medicines-related incidents or near misses in line with trust policy.

  • To promote the safe, secure, and cost-effective use of medicines.

Professional Development

  • To maintain your own professional competence and knowledge through continuous professional development (CPD).

  • To participate in training and development programmes provided.

  • To provide supervision and training to other pharmacy staff, pre-registration trainees, and students as appropriate.

    Additional Information

    • This is a temporary locum position to cover staffing gaps.

    • A full induction and on-site training will be provided.
