Main duties of the job
The post-holder will be a specialist lead in the assessment and case management for Children and Young People (CYP) with complex health needs. You will be responsible for coordinating the NHS Continuing Healthcare (CHC) process, which includes chairing multi-disciplinary team meetings to determine eligibility, developing and reviewing personalised care plans, and managing the allocated budget for individual packages of care. A key duty will be to provide expert clinical advice, ensure robust safeguarding processes, and work collaboratively with families, carers, and partner agencies like education and social care to achieve the best possible outcomes for the child or young person.

Main Responsibilities:
Clinical Assessment & Decision Making:
To act as the nominated lead, coordinating and undertaking comprehensive assessments for NHS Continuing Healthcare in line with the National Framework.
To facilitate Multi-Disciplinary Team (MDT) meetings, ensuring the effective completion of the Decision Support Tool (DST).
To apply professional and clinical knowledge to assess the nature, complexity, intensity and unpredictability of an individuals health needs.
To produce clear, concise, and rationalised reports and recommendations for panel, justifying decisions based on evidence.
Caseload Management:
To proactively manage a defined caseload, ensuring all assessments, reviews, and case notes are maintained to a high standard and within statutory timeframes.
To undertake regular reviews of care packages and eligibility for individuals in receipt of CHC funding.
Stakeholder Engagement & Communication:
To act as a key contact and source of expertise for individuals, families, and carers, explaining the CHC process with clarity, sensitivity, and empathy.
To work collaboratively with a wide range of health and social care professionals, including GPs, Social Workers, and hospital consultants.
To provide professional advice and support to colleagues within the ICB and partner organisations.
Professional Development:
To maintain your own professional registration and competence, taking responsibility for continuous professional development.
To contribute to the training and development of junior staff and peers.
To participate in clinical and caseload supervision.
