We are looking for an organised, self-motivated
and confident individual to join our Social Prescribing team, helping adult and
child patients to access support that will help them lead more independent,
resilient and healthier lives.
Main duties of the job
Working alongside GP practices, Social
Prescribing Link Coordinators help patients and families connect
with health and community services which will make a real difference to
their emotional and physical wellbeing.
You will deliver personalised support to
patients, independently managing new referrals from health professionals
and a caseload. It is essential that you have an understanding of how to
prioritise need and are able to recognise when to refer on to specialist
services, being confident to communicate at all levels across
health and social care sectors.
The
position is available in north Kent and applicants must have experience of
working with adults or children, a background in medical or social care, and
experience of delivering health and wellbeing support to clients or groups.
You will need to
demonstrate a robust knowledge of confidentiality, data protection,
safeguarding and boundaries in all aspects of your work.

Job description
Job responsibilities
- Independently manage GP referrals,
self-referrals and online enquiries for people with health conditions
(including common mental health conditions, obesity, diabetes, respiratory
conditions, mobility issues and sensory impairment)
- Work with children, families and patients
in a supportive, holistic way to address practical and psychological barriers,
such as lack of transport, low confidence and social isolation, coping
strategies for daily living; work together to co-produce a solution
- Support communication with young people and
adults, their families and between services; advocate for young people and
adults at risk
- Make referrals to local community groups
and other agencies; facilitate the sharing of information, resources and
intelligence about any gaps in provision
- Accurately represent the PCN and ensure the
values of the PCN are always upheld in carrying out the role
- Develop the role in line with the
requirements of the PCN; undertake activities to meet delivery needs; work with
clients on a one-to-one basis; deliver group support where required
- Develop networks and work in partnership
with other agencies; attend multi-disciplinary meetings
- Signpost and facilitate access to support
services including financial and benefit advice, housing, health and safety
home systems, and leisure and social activities
- Work with GPs and health and social care
professionals to increase awareness of the services; attend local and
networking events
- Take part in comprehensive induction,
training and development, adhere to Imago policies, and work with an
understanding of lone working, data protection, and health and safety
principles
