We are looking for someone to support people to take
positive steps to improve the way they manage their mental wellbeing and
day-to-day lives, based on what matters most to them.
Main duties of the job
Taking referrals from GP practices, you'll work with
child and adult mental health patients to identify what is affecting their health and
wellbeing. You'll then work with them to create a personalised support plan to
improve resilience, health and wellbeing. You'll find services and activities
that offer practical, social and emotional support, working as a team with
other healthcare professionals who may provide more traditional medical
interventions.
This might mean connecting people with art classes,
walking groups or gardening clubs, or it could be signposting to more practical
support, such as services providing advice on debt or housing issues. Its
essential that you can listen carefully, build relationships, and motivate
people to make choices and changes about the things that matter to them.
The position is based in Sheppey and applicants must
have experience of working with adults or children in a medical or social care
setting, and of providing mental health support. You should be organised,
self-motivated and able to work as part of a team and on your own. Car driver

Job responsibilities
- Independently manage GP referrals, self-referrals and online enquiries for patients with mental health conditions
- Provide holistic support to patients to address practical and psychological barriers, such as lack of transport, low confidence and social isolation, coping strategies for daily living; work together to enable positive change
- Support communication with patients and their families, and between services; advocate for patients
- Make referrals to local community groups and other agencies; facilitate the sharing of information, resources, and intelligence about any gaps in provision
- Signpost and facilitate access to support services including financial and benefit advice, housing, health and safety home systems, and leisure/ social activities
- Develop the role in line with the requirements of the PCN; undertake activities to meet delivery needs
- Work with clients on a one-to-one basis; develop and deliver group sessions to patients and families where needed; develop local peer support networks
- Work closely and independently within individual practices in Primary Care Networks; accurately represent the PCN and ensure the values of the PCN and Imago are always upheld
- Develop networks and work in partnership with other agencies; attend multi-disciplinary meetings

- Take part in comprehensive induction, training and development, adhere to Imago policies, and work with an understanding of lone working, data protection, and health and safety principles

- Relevant professional experience of working with vulnerable adults in social care, health, education or voluntary and community sector
- Experience of delivering mental health support to individuals or groups, and of working with individuals to identify and action personal goals
- Proven experience of report writing, and monitoring and evaluation
- Operational experience of multi-agency working
- Robust understanding of safeguarding roles and responsibilities
- Ability to work independently and as part of a team; to meet objectives, achieve targets and work to tight deadlines
- Excellent communication skills written and verbal
- Ability to stay calm in challenging situations and have a measured response
- Confident working within professional boundaries; ability to forge effective relationships at all levels
- Proficient in using Word, Excel, websites; experience of using a database
- Good level of education
- Valid UK driving licence and access to own vehicle
