One Wirral CIC is delighted to offer an exciting opportunity for a skilled and compassionate Practice Nurse to join our team, providing high-quality primary care services at Leasowe Medical Practice. This role is ideal for a motivated nursing professional who is committed to delivering safe, effective, and patientcentred care within a supportive and forwardthinking environment.

As part of our communityfocused organisation, you will play a key role in improving health outcomes for the residents of Leasowe. You will work alongside an experienced multidisciplinary team, contributing to the delivery of holistic care, supporting longterm condition management, and promoting health and wellbeing across our patient population.

Main duties of the job

  • Deliver core nursing services including wound care, immunisation, cytology, health checks, and clinical assessments
  • Lead and manage long-term condition reviews e.g. diabetes, asthma, COPD, hypertension), creating personalised care plans and supporting patient self-management
  • Provide health promotion , lifestyle advice, and preventative interventiosn to improve population health
  • Work collaboratively with the multi-disciplinary team, contributing to coordinated care, quality improvement, and service development
  • Maintain high clinical standards through safe practice, accurate documentation, adhearence to governance processes, and ongoing professional development

Clinical Duties

Provide assessment, screening, treatment, and health education services to the practice population.

Deliver chronic disease management reviews including:

  • Diabetes
  • Hypertension
  • Asthma
  • COPD
  • CHD

Undertake treatment room duties including:

  • Wound care and dressings
  • Ear irrigation
  • Cervical cytology
  • Immunisations and vaccinations (adult and childhood)
  • ECGs
  • Blood pressure checks
  • NHS Health Checks

Provide travel health advice and vaccinations (as appropiate)

Support the delivery of national screening and immunisation programmes.

Support patients with longterm conditions through personalised care planning and lifestyle advice.

Recognise and manage acute presentations appropriately, seeking senior clinical support when required.

Theraputic Monitoring including INR Management

  • Checking compliance with and adherence to appropriate treatments using an holistic patient-centered approach
  • Recognising abnormalities and referring to doctors appropriately.
  • Identifying the impact of treatment and implementing or recommending changes as appropriate

Infection Control

  • Support patients and colleagues in adopting sound infection control measures
  • Apply infection control measures within the practice according to local and national guidelines
  • Carry out random infection control audits within the practice.

Professional Responsibilities

  • Work within NMC Code and scope of professional practice.
  • Maintain accurate, legible, contemporaneous electronic patient records using EMIS Web.
  • Participate in clinical audit, significant event analysis, and quality improvement activities.
  • Contribute to the achievement of QOF, DES and IIF, and local targets.
  • Engage in safeguarding processes, recognising and escalating concerns relating to adults and children.
  • Maintain uptodate clinical knowledge, mandatory training, and competencies relevant to primary care.
